Description:
The course is deigned to give delegates an appreciation of the importance and value of learning from accidents and incidents, and to give delegates the knowledge and skills required to investigate such events and successfully identify and address the root causes thereby reducing the risk from further events.
Training Purpose:
By the end of the workshop delegates will be able to:
- Outline the benefits of accident investigation in the safety management cycle
- Identify the correct reporting requirements for any given accident/incident
- Select an appropriate level of accident investigation in relevance to the accident/investigation
- Explain the lifecycle of an accident/incident
- Describe the key stages of accident investigation
- Identify the information needed to be collected in relation to an accident investigation
- Analyse information collected and differentiate between fact and supposition
- Interpret the findings of the accident investigation to identify root causes
- Define appropriate remedial actions to reduce the risk of a recurrence
